Prime Community Care is a registered NDIS service provider which offers a wide range of disability support options to ensure you can lead an independent lifestyle. Prime Community Care delivers exceptional services and is available to assist clients with support 24 hours per day, 7 days a week. We work with you to make sure we are providing you with the best possible care tailored to your plan.
Our dedicated team is committed to providing a high level of support and services to people with special needs and disability of all ages at home and in the community.
We provide Support Workers and other professionals to enable our clients to live as independently as possible. We believe in working closely with the individual, their family and carers, professionals, and other stakeholders in their life, to deliver high quality support services suited to their unique needs.
